Treatment of glaucoma in the form of surgery should be the last resort. The therapy begins with the administration of special eye drops, thanks to which it is possible to effectively reduce the pressure in the eye, and this will prevent the disease from developing. In some cases, however, such treatment is not sufficient or cannot be taken. Then the solution will be surgical treatment.
It is worth checking what are the indications for surgical treatment and what methods can be used here. As already indicated, surgical treatment is the last resort. It will be a treatment that is carried out at the moment when pharmacological and laser treatment is no longer sufficient and it has not managed to stop the development of the disease and thus damage to the optic nerve.
In addition, surgical treatment is considered when it comes to juvenile glaucoma and there is a large fluctuation in the form of intraocular pressure changes. Then other treatments can affect the quality of life. Sometimes there may also be a problem of intolerance of the drops used. Only in certain surgical situations will the treatment be the first choice. Especially with glaucoma of closing angle with complete or almost complete closing angle, when it is not possible to control the pressure. In addition, in a situation where blood pressure will have to be lowered significantly and it will not be possible to achieve this with other forms of treatment.
Surgical treatment of congenital glaucoma
When it comes to congenital glaucoma at the beginning, a goniotomy is performed, which will consist in cutting the angle of seepage, so that it is possible to lead to decongestion.
Treatment of the second wave will consist in performing the treatment, which will be the trumpetkulectomy of m. Cairns. This is a treatment that involves creating a new canal in the eye to prepare a suitable area where aqueous liquid will drain off between the anterior chamber and the space between the Tenon's handbag.
The third treatment is a seton procedure, in which special plastic implants will be prepared, which allow to create a new connection between the front chamber and the space under the Tenon's handbag. The alternative may also be special cyclodestruction procedures, the task of which will of course be to reduce the production of liquids, and this will ensure that the pressure in the eye can be significantly reduced.
These can be such procedures as: cyclodiatrics, cyclophoto regulation by diode laser, cyclocrioaplication, endocyclo photoagulation by diode laser. In case of surgery, trabeculectomy will be performed and in the next cast seton and cyclodestruction procedures will be performed.
Surgical treatment of primary open angle glaucoma
1. initial to intermediate glare:
- Non-perforating procedures, thanks to which the aqueous liquid flows away and the front chamber does not need to be opened.
- canaloplasty
- trabekulektomy
2. medium to advanced glare:
- non-perforating procedures
- canaloplasty
- trabekulektomy
- seton treatments
- laser cyclophotocoagulation procedures of the epidural or endoscopic laser
Operation of secondary open angle glaucoma - the procedure here is similar to that in the middle or more advanced open angle glaucoma. The surgical treatment of glaucoma of the closing angle is also possible:
- fistulas
- trabekulektomy
- seton treatments
- Cyclodestruction procedures
Therefore, it is clear that currently glaucoma treatment is possible in various forms and thanks to that, overcoming this problem should not be a major problem.
